We, the undersigned, do hereby make application for exhibit space as indicated below. We have read and agree to comply with the exhibit regulations and instructions. A deposit of $150 will be paid to reserve a space in either the virtual or in person convention. We agree to pay the balance of the space rental price when billed. If it is necessary for us to cancel, we understand that 50% of the advance payment may be refunded before space is assigned and no refund will be made after space has been assigned. We further agree that we shall not be permitted to set up our display until our exhibit fees have been paid in full. We understand that the early bird exhibit booth fee is $300 and is due by December 1, 2020. The fee of $325 is due before January 1 and a $350 late fee will be due after January 1 2021. For fees paid after January 1, the company name will not appear in the Conference program. THE $350 LATE FEE MUST BE CASHIER’S CHECK, MONEY ORDER OR CASH. Special note: Early bird fee for a vender with three or more booths is $250 per space by December 1. The fee will be $275 after December 1 and before January 1. After January 1 the late fee is $300 per space.
